Registration form

Registration for NFIC 2010 is now open.

In order to attend the workshop, registration is required.
On-site registration is possible, however, pre-registration is preferable.
Registered participants will receive ID badges and conference bags.
No bagde, no entry !

Emplyees of the Institute of Cardiology of the Jagiellonian University are welcomed to participate, however, they need to fill in the registration form below.

Students, members of the Students’ Scientific Societies affiliated at the Institute of Cardiology of the Jagiellonian University are welcomed to participate (December 1-4, 2010), provided that they send student lists to nfic2010@nfic.pl before November 15, 2010. Only badges will be provided during the Workshop.

IMPORTANT NOTICE

Registration for CTO workshop on December 1st, 2010 (Wednesday) is limited by the number of places available.

Friday-Saturday (December 3-4, 2010) NFIC workshop - Physicians: 300 Euro (1000 PLN), Nurses, technicians: 0 Euro (0 PLN)
